Suffocation released Pierced From Within in 1995. The album's artwork is a nightmarish scenario tinted dark green with the band's logo in yellow and red overtop. This is not how the original artwork is colored.
In a recent report from MetalSucks, someone "close to the creation of the album" has admitted something went wrong with the coloring at some point. The original artwork done by Hiro Takahashi is more of a light blue, as you'll see from the two photos below.

MetalSucks also walks readers through the process of how album art is created.
An artist is hired to create an image. That original piece of art is then turned into something called a “transparency,” which a designer then uses to make the final cover as we know it (e.g., with the band’s logo, the album title, etc.). Some artists make their own transparency, which they then give to the label, presumably so as not to risk losing the original piece. Others, however, do, in fact, send their original art to the label, who then make the transparency before shipping the art back to the artist.
